Shaam-e-gham,
kuch us "nigaah-e-naaz" ki,
baaten karo............
Bekhudi badhti chali hai
raaz ki baaten karo............
"Nakhat-e-zulf-e-pareshaan ",
dastaan-e-shaam-e-gham,
Subah hone tak,
isi andaaz ki baaten karo.......
Ye sukut-e-yaas,
ye dil ki ragon ka tootna,
Khamoshi mein kuch,
shikast-e-saaz ki baaten karo..........
Kuch qafas ki tiliyon se,
Chhan raha hai noor sa,
Kuch faza,
kuch hasrat-e-parvaaz
ki baaten karo.....

The literally meaning.......
Shaam-e-gham,
kuch us "nigaah-e-naaz" ki
baaten karo.......
Shaam : evening
gham : desperation
Kuch : some
Us : her my beloved
Nigaah : eye's glare
Naaz : pride
Baaten : talk about
An evening of desperation Please talk about "glare of my eyes pride" .......
Bekhudi badhti chali hai
raaz ki baaten karo.......
Bekhudi : Restlessness
Badhti : increasing
Chali hai : continuously
Raaz : secret
Baaten : talk about
Restlessness is increasing, hence talk about
secrets (of my beloved ) .........

isi andaaz ki baaten karo.......
Nakhat : Fragrant
Zulf : Hair
Pareshan : Disarrayed Dastaan : story
shaam : evening
gham : desperation
Subah : morning
Hone tak : till morning 
isi : only this
Andaaz : style
Baaten : talk about
The poet want to shed his desperation, in a lonely evening,, by talking with himself about the fragrant hair of his beloved, that is why, he is requesting the loneliness to continue the talk of his beloved till morning ....
Yeh sukut-e-yaas,
yeh dil ki ragon ka tootna,
Khamoshi mein kuch,
shikast-e-saaz ki baaten karo....
Yeh : this
Sukut : silence
yaas : despair
Yeh : this
Dil : heart
Ragon : nerves
Tootna : breaking
Khamoshi : silence
Main : in
Kuch : some
Shikast : defeat
Saaz : rhythm ...
In This "silence of despair" & ( with breaking of my heart's nerves ) , in this silence talk about defeat of my rhythm......
Kuch qafas ki tiliyon se,
Chhan raha hai noor sa,
Kuch faza,
kuch hasrat-e-parvaaz
ki baaten karo.....
qafas : cage
Tiliyon : cage bar 
Faza : Fiza : atmosphere
Hasrat : desire
parvaaz : flight high altitude
Some light rays filtering through the prison's  window's iron bars ( tiliyon )  , the poet imagination is to fly like a  bird in open air with great height to meet his beloved love.

The accurate meaning....
The poet is so lonely and desperate in an absolutely silent evening in a prison , missing his beloved so much.
This loneliness is like a cage or a prison for him, to avoid this restlessness, he want to remember his beloved, the beautiful things about her secrets with him , her Fragrant hair.
He further want to tell her, about his heart break in her absence, about the defeat of his rhythm.
Ultimately, he want to liberate himself like the rays of light coming freely from the prison windows ( to get rid of loneliness ) , so that he can fly high to meet his beloved....
